The purpose of Social Services is to provide services to patients and their families to promote optimal social function and to enable them to utilize available resources to achieve their optimal level of health. Social Services can assist the patient and family in utilizing community resources, health care services, and his/her own resources in treatment of an illness. Decatur Health will provide medically-related Social Services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each patient.
• Making arrangements for obtaining needed adaptive equipment, clothing, and personal items
• Assist the patient or family with social, emotional, and family problems precipitated by illness and hospitalization
• Making referrals and obtaining services from outside entities
• Assisting patients with financial and legal matters and information regarding Medicare, Medicaid, Health Insurance Marketplace and medication assistance programs
• Discharge planning services, helping to place patient in nursing home, arranging for home care services
• Encourage optimum utilization of community resources and health care
• Serve as liaison between hospital and resources in the community
• Arranging provision of needed counseling services
• Assist patients to determine how they would like to make decisions about their health care, and whether or not they would like anyone else to be involved in those decisions

Cherish Seifert is a Licensed Bachelor Social Worker who attended and graduated from Metropolitan State University in 2013. She has over ten years of diverse experience in helping vulnerable populations.
